plus heparin IV rebolus for ACS as needed for aPTT <37.6 sec (per protcol-which is automatically set in the computer) from infusion bag 4510 units IV every 6 hrs PRN (they would draw aPTT every 6 hrs)
plus heparin IV rebolus for ACS as needed for aPTT 37.6-42.1 sec from infusion bag 2,260 units IV every 6 hrs PRN
